❑3 axles (GVWR 40,000 – 55,000 pounds)

❑4 or more axles being operated in Allegany and Garrett Counties only (GVWR 40,000 – 70,000 pounds)

❑4 or more axles in compliance with lift axle regulations being operated outside of Allegany or Garrett Counties.

You must attach manufacturer’s certiication. (GVWR 40,000 – 70,000 pounds)

I/we certify the vehicle is designed to haul feed or other loose materials in bulk, is self-unloading by gravity or mechanical means and the load will be enclosed.

Loose Material - Feed, dirt, sand, wood chips, stone, gravel, aggregate, concrete, or other similar material that can blow, fall, or spill from a vehicle as a result of movement or of exposure to air, wind or weather. Liquid loads are not included as loose material. Bulk - The total load is in a mass, not divided into parts or packaged in seperate units and can not be counted. Containerized cargo is not included.

Self-Unloading by gravity or mechanical means - Once the unloading process is started, gravity or mechanical means takes over and no further action or attendance by an operator is necessary to complete the unloading process.

This application requires the signature of the owner(s).

•If jointly owned, all owners signatures are required.

•If the owner is a business entity, the person legally authorized to sign must state their capacity after their signature.

•If the owner is a trust, the trustee must sign and state their capacity.

1.MARYLAND REGISTERED 4-AXLE DUMP SERVICE VEHICLES CURRENTLY REGISTERED FOR 70,000 POUNDS:

A.Must have lift axle certiied to be in compliance with the regulations. Certiication must be by a licensed manufacturer/second stage manufacturer.

B.The certiication must include the PSI setting to equal the minimum acceptable lift axle weight of 12,000 pounds when the vehicle has a GVW of 70,000 pounds.

C.The manufacturer/second stage manufacturer must provide you with documentation of the certiication.

D.You must submit to the Motor Vehicle Administration a copy of the certiication documentation, accompanied by a photograph of the lift axle.

E.You must carry the certiication documentation in the cab of the vehicle at all times and must present it to the law enforcement upon request.
