Accommodation Third Party Form PDF Details

In an effort to ensure equality and facilitate access for all, the Accommodation Third Party form stands as a crucial tool for applicants, residents, or participants requiring adjustments in their living conditions due to disabilities. This document, which follows the Request for Reasonable Accommodation, provides a structured way for an independent verifier, typically a medical professional familiar with the individual's condition, to confirm the need for specific accommodations without delving into personal medical details. The form encapsulates critical information such as the applicant's name, client number, and the nature of the impairment, focusing on how it limits major life activities or bodily functions, and how the requested accommodation would enhance the individual's enjoyment and functionality within the housing program. Located in Cook County, the form's guidance helps to navigate the intersection of personal needs with federal and state disability laws, ensuring that the accommodations considered do not impose undue financial or administrative burdens. This document not only underscores the importance of a meticulous and compassionate approach to housing accommodations but also highlights the legal and procedural framework in place to support individuals with disabilities, reflecting a commitment to inclusivity and equal opportunity.

The following questions may help determine whether the requested accommodation is needed because of the disability.
