The Arkansas Acknowledgment of Paternity form is the official legal document used to establish paternity when parents are not married. This form cannot be signed until after the child is born. Parents must complete it in black ink with no corrections or alterations.
Both parents must sign the acknowledgment of paternity form before a licensed notary. Each parent must present valid government-issued identification when signing. Both parents must also provide their Social Security Numbers. Federal law requires Social Security Numbers on the form for child support enforcement purposes.
Parents must list health insurance information on the form even if the insurance does not cover the birth expenses. Completing and submitting the form at the hospital after the birth avoids a separate filing fee. A charge still applies for obtaining a certified copy of the birth certificate.
Filing the form after leaving the hospital requires notarization and submission to the Arkansas Department of Health Vital Records office. Parents wishing to change the child’s last name must follow additional instructions when filing the form. If the mother was married during pregnancy but the husband is not the biological father, the form includes a section for the husband to deny paternity.
