Fole Form 2137 PDF Details

Ensuring watercraft are free from the threat of aquatic invasive species (AIS) is a paramount concern for both environmental and recreational reasons. The Arizona Game and Fish Department's Aquatic Invasive Species Boat Inspection Report, known as the FOLE 2137 form, serves as a comprehensive tool in this battle, guiding owners through a detailed self-inspection process. It incorporates protocols recommended by the Western Regional Panel on Aquatic Invasive Species to detect the presence of AIS, particularly invasive mussels, on watercraft. The form requires detailed information about the vessel, including type, ownership, make, model, and size, as well as specifics regarding the inspection circumstances such as the date, location, and weather conditions. Crucial steps include a close inspection of various parts of the boat for visible AIS, with particular attention to mussels and vegetation. If detection occurs, the form also outlines a decontamination plan, specifying actions such as cleaning, draining, and drying the boat according to established procedures. Additionally, it contains a legal acknowledgment by the person conducting the inspection about the accuracy of the information and the understanding of legal responsibilities concerning the transport of watercraft potentially harboring AIS. This form not only plays a vital role in preventing the spread of invasive species across water bodies but also ensures compliance with Arizona's stringent environmental protection laws.

How:

I affirm that the above information is true. I understand that this inspection form does not certify that the watercraft listed herein has been completely decontaminated; only that it was self-inspected by the owner/transporter for presence/absence of visible aquatic invasive species (AIS), including invasive mussels. I understand the vessel may not be transported from the inspection

site or other location as authorized by the Department until such time as vessel has been properly cleaned, drained and dried for all aquatic invasive species, per current AGFD Director’s Orders. I further understand that it is unlawful to transport Arizona

listed AIS (e.g., quagga mussels) under A.R.S §17-309 and §17-255.02, and Arizona Administrative Code Rule R12-4-406.

Arizona Game and Fish Department AIS Boat Inspection Report (AISBIR)

Per Watercraft Inspection Protocols Recommended by Western Regional Panel on Aquatic Invasive Species

Comments/notes:

Long Term Moored Boat Decontamination Procedures:

Step 1. Clean/remove any clinging material such as plants, animals and mud from anchor, boat, motor, equipment and trailer.

Step 2. Remove the plug (if applicable) and drain the water from the bilge, live-well and any other compartments that could hold water. Drain water from engine and engine cooling systems(s).

Step 3. Physically remove all visible attached mussels from boat surfaces, motors, impellers, outdrives, rudders, anchor(s) and through hull fittings.

Step 4. Flush engine and cooling system and any other through hull fittings with hot water that is exiting those areas at 140 degree Fahrenheit for ten to thirty seconds.

Step 5. Keep the boat out of the water and dry (desiccation) all areas of the boat, including bilge, through

hull fittings and engine, for a minimum of 18 consecutive days during the months of November through April and 7 consecutive days from May through October.

Step 6. Completely fill out and file the required Aquatic Invasive Species Boating Inspection Report (AISBIR). This form can be downloaded at www.azgfd.gov/ais.
