Ensuring the health and safety of students is a top priority for educational institutions, which is why the State of Connecticut Department of Education mandates the completion of the Health Assessment Record (HAR-3) for all students entering school, as well as at key points during their educational journey. This comprehensive form is split into two main parts: Part I is to be filled out by the parent or guardian and covers the student's health history, including immunizations, hospitalizations, allergies, and other significant health-related information. Part II requires a detailed medical evaluation from a qualified healthcare provider, including physical examination findings and an updated immunization record. The HAR-3 form not only ensures compliance with Connecticut's state laws regarding health assessments and immunization but also facilitates the monitoring and support of students' health needs within the school setting. This requirement underscores the state's commitment to maintaining a healthy and conducive learning environment for all students by preemptively addressing health concerns that could impede their educational experiences.
