To renew your vehicle’s registration in conjunction with this application, you must complete and attach your vehicle renewal application or Form MV-140, “Request for Registration,” and a check or money order, made payable to the Commonwealth of Pennsylvania, for the appropriate annual registration renewal fee in addition to the appropriate plate fee.

AU.S. military registration plate cannot be duplicated. If your plate is lost, stolen or defaced, you will need to reapply for a U.S. military plate bycompletingthisapplicationandsubmittingwiththeappropriatefees.Shouldyouneedanimmediatereplacementregistrationplate,please complete and submit Form MV-44, “Application for Duplicate Registration Card, Replacement of Registration Plate, Renewal Sticker or Weight Class Sticker,” and submit a fee of $7.50. A standard plate will be issued until the replacement military plate can be provided.

REQUIRED DOCUMENTATION

Veterans who cannot provide the required documentation below should contact their nearest VA Office and obtain Form SF180, “Request Pertaining to Military Records,” which should be completed and forwarded to the appropriate Personnel Records Center. Form DD214, "Separation Papers or Discharge Papers," must be certified by the County Recorder’s Office located in the county courthouse.

1.AnEx-PrisonerofWarPlateapplicantmustsubmitalegiblecertifiedphotocopyoftheCertificationofImprisonmentfrom the appropriate branch of theArmed Forces or from the VeteransAdministration, or a certified legible photocopy of Form DD214, "Separation Papers or Discharge Papers," which indicates dates of imprisonment. Registration Fee - $10 processing fee in lieu of applicable full registration fee.

2.A Purple Heart Plate applicant must submit a legible certified photocopy of Form DD214, "Separation Papers or Discharge Papers" or one of the following, which clearly indicates that a Purple Heart was awarded:

u A copy of the general order.

u A copy of the Certificate of Award.

u A certification of the award from the Veterans Administration.

3.An Expeditionary ForcesPlate applicant must submit a legible certified photocopy of Form DD214, ”Separation Papers or Discharge Papers,” which indicates that either a Naval or Armed Forces Expeditionary Medal, a Kosovo Campaign Medal or a Global War on Terrorism Expeditionary Medal was awarded.

4.A Korean Defense Service Plate applicant must submit a legible certified photocopy of Form DD215, “Correction to DD214,” indicating Korean Defense Service Medal was applied for and awarded.

5.AKorean War Veteran Plate applicant must submit a legible certified photocopy of Form DD214, "Separation Papers or Discharge Papers" or one of the following, which indicates the applicant served in Korea anytime during the period of June 27, 1950, through Jan. 31, 1955:

u A copy of the Korean Service Medal Award.

u Statement of Service in Korea during the period of time listed above from the St. Louis, Missouri, Records Center at: Department of Army, National Personnel Records Center, 9700 Page Blvd., St. Louis, Missouri 63132.

6.AnOperationEnduringFreedomPlateapplicantmustsubmitalegiblecertifiedphotocopyofFormDD214,“SeparationPapers or Discharge Papers,” indicating Operation Enduring Freedom service or anAfghanistan Campaign Medal was awarded.

7.An Operation Iraqi Freedom Plate applicant must submit a legible certified photocopy of Form DD214, “Separation Papers or Discharge Papers,” indicating Operation Iraqi Freedom service or an Iraqi Campaign Medal was awarded.

8.A Persian Gulf War Veteran Plate applicant must submit a legible certified photocopy of Form DD214, "Separation Papers or Discharge Papers," or one of the following, which indicates service beginning Aug. 2, 1990, in the Southwest Asia Theater of Operations to present time:

u A copy of the Southwest Asia Service Medal awarded between Aug. 2, 1990, through Nov. 30, 1995, or,

u A copy of the Armed Forces Expeditionary Medal awarded between Dec. 1, 1995, to the present or, when the applicant is currently on active duty, a statement of service from the applicant’s commanding officer verifying duty in the Southwest Asia Theater.

9.A U.S. Air Force Veteran Plate, U.S. Army Veteran Plate, U.S. Coast Guard Veteran Plate, U.S. Marine Corps Veteran Plate and U.S. Navy Veteran Plate applicant must submit a legible certified photocopy of Form DD214, "Separation Papers or Discharge Papers," which indicates honorable discharge from the Armed Forces or a legible certified photocopy of the front and back of their DD2(RET), "United States Uniformed Services Identification Card (Retired)" or a legible certified photocopy of the front and back of their DD2S(RET), "United States Uniformed Services Identification Card (Retired)."

10.A Vietnam War Veteran Plate applicant must submit a legible certified photocopy of Form DD214, "Separation Papers or Discharge Papers," which indicates one of the following medals was awarded:

u AcopyoftheArmedForcesExpeditionaryMedalifapplicantservedinVietnambetweenJuly1,1958,toJuly3,1965, or,

u AcopyoftheVietnamServiceMedalifapplicantservedinVietnambetweenJuly3,1965,throughMarch28,1973.

11.An Airborne Unit Plate applicant must submit documentation that indicates they satisfactorily completed the prescribed proficiency tests while assigned or attached to anAirborne Unit and any other military personnel determined by PennDOT to be appropriately classified as a member of a United States Military Airborne Parachutist or Glider Unit. NOTE: U.S. MilitaryAirborne Units include Parachutist or Glider Units, Glider Troopers, Paratroopers,AirAssault Troopers, Rangers, U.S.Army Special Forces, USMC Recon, U.S. Navy Seals, U.S.Air Force Special Operations, Troop Carrier Command, including Glider Pilots and the 160th SOAR.

12.A Silver Star, Bronze Star for Valor, Distinguished Service Cross, Distinguished Flying Cross, Navy Cross, Air Force Cross and Bronze Star applicant must submit a legible certified photocopy of Form DD214, “Separation Papers or Discharge Papers,” indicating the requested medal was awarded.

13.A Veteran Plate or a Veteran Motorcycle Plate applicant must submit a legible certified photocopy of Form DD214, "Separation Papers or Discharge Papers, or a legible certified photocopy of the front and back of their DD2(RET), "United States Uniformed Services Identification Card (Retired)" or a legible certified photocopy of the front and back of their DD2S(RET), "United States Uniformed Services Identification Card (Retired)." which indicates honorable discharge from theArmed Forces of the U.S. or a Reserve component of theArmed Forces."
