A Statutory Declaration Re Proof of Death completed and signed by the surviving joint tenant or anyone who is acquainted with the facts. The statutory declaration must be sworn before a Commissioner for Oaths who must print his/her name and expiry date or a Notary Public who must affix his/her seal.

An original Certificate of Death issued by Alberta Vital Statistics (the original certificate will be returned).
