Who must use this form?

You must submit the voucher below (Form TMT-2.9-V) with your

payment when you have a balance due and you use the One

Stop Credentialing and Registration (OSCAR) system to renew your highway use tax (HUT) and automotive fuel carrier (AFC) certiicates of registration (C of R).

Attach a check or money order payable in U.S. funds to

Commissioner of Taxation and Finance. Include on your check or money order your identiication number, Form TMT-2.9-V, and your HUT/AFC renewal transaction

number.

Note: Include a fee of $19 for each truck, tractor, or

self-propelled device, and for each trailer, semitrailer, dolly, or

other device that transports automotive fuel. Each vehicle must have a HUT/AFC Certiicate of Registration and must display a decal. A decal must be displayed on each vehicle by January 1,

2013.
