Form Uic 25 PDF Details

The UIC-25 form serves as a critical document for those seeking to obtain a permit for Class-V wells in Louisiana, under the regulation of the Louisiana Department of Natural Resources, Office of Conservation, Injection & Mining Division. It encompasses a comprehensive range of data specifications, from basic well identification and ownership details to technical aspects such as well casing, cement data, and injection zone depths. Applicants are required to specify the application type, which includes options for drilling and completing a new well, converting an existing well, permitting an existing well, or other specified activities. The form also requires detailed information about the well's physical location, ownership details, intended use, and technical specifications such as latitude, longitude, and Louisiana Coordinate System coordinates. Moreover, the form mandates the disclosure of any permits, licenses, or approvals related to the applicant's legal or technical ability to pursue the proposed activity. Furthermore, the document outlines specific operational and construction standards and provides guidelines for completing the application package, which includes submitting the necessary fees, supporting documents, and technical descriptions. This detailed approach ensures that Class-V well operations meet state guidelines, protecting natural resources and public health.
