In the journey towards more accessible transportation solutions, the General Motors (GM) Mobility Reimbursement Application plays a pivotal role. This comprehensive form, spread across two pages, provides a clear pathway for individuals with permanent disabilities to enhance their mobility through vehicle adaptations. It underscores GM's commitment to ensuring mobility for all by offering financial assistance towards the cost of eligible adaptive equipment. Vital details such as step-by-step instructions, a delineation of eligible equipment, and the importance of utilizing licensed installers underscore the form's aim to facilitate a smoother modification process. To qualify, applicable equipment must be permanently installed on new and unused Chevrolet, Buick, and GMC vehicles from model years 2011 to 2013 within specific time frames. Moreover, the form elaborates on the documentation required to validate claims, including an itemized invoice of the installation and a physician’s note for certain modifications. The inclusion of a maximum reimbursement amount, coupled with an additional OnStar offer for eligible vehicles, highlights GM’s dedication to offering both financial support and enhanced safety features. With explicit instructions on submission and dealer verification, the GM Mobility Reimbursement Application sets a structured route for individuals seeking to modify their vehicles for better accessibility, ensuring that mobility limitations are not a barrier to independence.
