NOTE: Approval permits importation for up to one year. If additional time is required, a request for an extension must be submitted in writing along with a copy of the prior approval letter no later than 30 business days before the expiration date. Extensions may be granted for a total of up to 5 years. If permission is sought for more than one vehicle, attach an addendum with year, make, model, and VIN for each vehicle. All requests submitted to this office must allow 30 business days for determination. All sections of this form must be complete and submissions in writing must be legible (49 CFR Part 551.31). It is the importer’s responsibility to make sure all Box 7 approvals are current with NHTSA.
