If you have pleaded guilty to or been convicted of any of the above, or unbecoming conduct for educators as listed by the Ohio Department of Education, it is important that you contact the Administrator of Investigations, Office of Professional Conduct, Ohio Department of Education (614/466-5638) on your own or through an attorney in order to determine if you are ineligible for an Ohio teacher license prior to continuing to pursue a teaching license through Lourdes University. Note that pleading “no contest” is equivalent to pleading “guilty” and that the Ohio Department of Education may investigate court records that have been sealed or expunged.

I have read the list of offenses which may keep me from gaining a teacher licensure in the State of Ohio. I also am aware of, read, understand and meet the good moral character standard of the State of Ohio Revised Code (Rule 3301-21-01 of the Administrative Code).

I agree that, while enrolled at Lourdes University/Tiffin University, if any event should occur that would cause me to answer YES to any of the above questions, or if I am guilty of any offense listed as unbecoming conduct by the Ohio Department of Education, I will immediately notify the Dean of Lourdes University Department of Education or withdraw from the Teacher Preparation Program.
