Wildlife Rehabilitation Intake Form PDF Details

The Wildlife Rehabilitation Intake Form serves as a vital initial step in the process of caring for injured or orphaned wild animals. It is crafted with the utmost respect for both the individuals who find and rescue these animals, and the creatures themselves, emphasizing the shared commitment to wildlife conservation. The form serves as a bridge between the public and the specialized care provided by the only non-profit, licensed wildlife veterinary hospital in the Chicago area, underscoring the legal framework that safeguards wildlife. It delineates the U.S. Fish and Wildlife Service's regulations against keeping wild animals without proper licensing, highlighting the role of licensed rehabilitators in ensuring appropriate care. With sections capturing essential information such as the finder's contact details, the circumstance of the animal's discovery, and its current condition, the form is designed not only to streamline the intake process but also to foster responsible wildlife interaction. It sensitively addresses the need for community support through donations, which are pivotal for the continued care and rehabilitation of these animals. The form ultimately aims to ensure the welfare of the wildlife under the center's care while respecting the natural order and legal stipulations governing wildlife rehabilitation.

WILDLIFE REHABILITATION INTAKE FORM

The fact that you have taken time to stop and pick up an injured or orphaned wild animal demonstrates a deep commitment and sense of caring on your part and is to be valued and appreciated. It is important you are aware that we feel the same way, which is why we operate the only non-profit, licensed wildlife veterinary hospital in the Chicago area.

The U.S. Fish and Wildlife Service prohibits individuals from maintaining any wild animal in captivity, even temporarily, without a license. By law, people cannot own the wildlife that exists around them. People (like Dr. Becker) can become licensed to care for injured wildlife, but no one owns these animals. Licensed wildlife rehabilitators are trained to provide the correct housing, food and medical care for native wild animals. If the animal is damaged beyond repair, state and federal regulations mandate these animals be euthanized. If the animal can be released, once healed we will release it into a correct and safe habitat.

We are only able to care for this animal with your help. Without your donation to help, we cannot sufficiency care for this animal.
