Policy Information
Start at the top of the form. Fill in the correct policy number exactly as it appears on the Commercial General Liability declarations page.
Confirm the named insured and policy period before moving forward. This endorsement only modifies the existing CGL policy.
Complete the Schedule Section
Next, focus on the Schedule section. That’s the only part you actually fill in.
You’ll complete two fields:
• Name of Additional Insured Person(s) or Organization(s). Write the full legal name of the owner, lessee, or contractor you’re adding as an additional insured.
• Location(s) of Covered Operations. List the project site or job location where the named insured is performing work. Include street address, city, and state when possible. If coverage applies to multiple sites, list each one or reference an attached schedule.
If the required information already appears in the policy Declarations, you may see it referenced there instead of writing it again. But confirm it’s accurate before issuing the endorsement.
Confirm and Issue
That’s it for manual entries. The rest of the form contains preprinted language that defines:
• Ongoing operations only
• Contract-based coverage limitations
• No completed operations coverage
• Limits that cannot exceed contract requirements or policy limits
Before finalizing, compare the endorsement to the written contract. Make sure the additional insured name and job location match the agreement exactly.
Once completed, the insurer issues the endorsement and attaches it to the policy.
