The Department of the Army Form 4713 is used for enlisted soldiers to request a transfer to another duty station. The form is also used to request a change in military occupational specialty, or MOS. This form must be completed and signed by the soldier, and then approved by their unit commander. If the soldier is requesting a transfer to another duty station within the United States, they may do so through their chain of command. If the soldier is requesting a transfer to an international duty station, they will need to submit their request through their servicing personnel office. The Department of the Army Form 4713 should be submitted as soon as possible, so that proper arrangements can be made.

INSTRUCTIONS
Upon resignation, retirement or transfer, the original of this record will be furnished for the personal file of the volunteer and a duplicate will be maintained at the organization for at least three years. In case of transfer, a duplicate record will be furnished to the gaining organization upon request of the volunteer. Upon completion of the calendar year, the annual total will be recorded on DA Form 4162.
