What it is: DAS 142 is the Request for Dispatch of an Apprentice form.
Where and when to file: Do not send this form to DAS; this form is to the Applicable Apprenticeship Committee.
You may use this form to request dispatch of an apprentice from the Apprenticeship Committee in the craft or trade in the area of the public work.
List one occupation/craft per form.

How to fill out DAS 142, box by box
The date and the Apprenticeship Committee
Enter the Date.
Under To Applicable Apprenticeship Committee, enter the committee Name, Address, Tel. No. and Fax No.
Contractor Requesting Dispatch
Under Contractor Requesting Dispatch, enter the contractor Name, Address, License No., PWC Registration Number, Tel. No. and Fax No.
Project Information
Under Project Information, enter the PWC Project Number, Contract Number, Total Contract Amount and Sub-Contract Amount.
Enter the Name of the Project and its Address.
Dispatch Request Information
Under Dispatch Request Information, enter the Number of Apprentice(s) Needed and the Craft or Trade.
Enter the Date Apprentice(s) to Report, with 72 hrs. notice required, and the Time to Report.
Enter the Name of Person to Report to and the Address to Report to.

Take note of California Code of Regulations, Title 8, § 230.1 (a) for all applicable requirements regarding apprenticeship requests.
Except for projects with less than 40 hours of journeyworkers work, you must request and employ apprentices in no less than 8 hour increments.
