Who Uses This Form

Only licensed and certified vehicle inspectors enrolled in the Minnesota Mandatory Inspection Program may submit decal orders. To complete the form, inspectors must provide:

• Telephone number and company details
• Inspector certification number
• Decal year and quantity requested
• Payment method (check or credit card)

Ordering Timeline and Cost

Decals are available for the current calendar year and the preceding year. Starting December 1 each year, decals for the next calendar year become available for ordering. Each decal costs $2.00. Orders are non-refundable and non-exchangeable, so verifying the correct year and quantity before submitting is essential. Payment is accepted by check (payable to the STATE OF MINNESOTA) or credit card, though a service charge applies to card payments. Orders by email or fax are not allowed for security reasons.

Compliance Agreement

By signing the form, certified inspectors agree to place decals only on vehicles that legally pass mandatory inspection standards. Misuse of inspection decals may result in revoked inspection privileges. The form includes sections for the inspector’s signature, certification number, beginning and ending decal numbers, and transaction details.

After Your Order Is Submitted

Once the completed form is mailed with payment, the State Patrol Division processes the order and ships decals to the address provided. Allow several business days for processing and delivery. Inspectors should keep a record of the beginning and ending decal numbers from their order for audits and compliance reviews.
