Form 1260 12 PDF Details

In the landscape of government operations, ensuring the right people have the right access to software applications is crucial for efficiency and security. Form 1260-12, stemming from the Department of the Interior in October 2003, serves as a bridge to this need within the Bureau of Land Management (BLM). This form, designed to address and streamline the process of granting, modifying, or removing access to various BLM software applications, acts as a comprehensive tool for managing user permissions. Users, whether they are government employees or contractors, must provide detailed information, including their name, organizational details, and the specific access requirements they are requesting. The process emphasizes accountability and security, necessitating users to affirm their responsibility for adhering to BLM computer system guidelines via Form 1264-3 prior to submitting Form 1260-12. Furthermore, the form facilitates a process of verification and approval, involving several tiers of management including supervisors, IT security managers, and application or program representatives. This layered approach ensures that access is granted appropriately and is aligned with organizational roles and needs. Completing and submitting Form 1260-12 is just the beginning; the actions taken by the Land and Resources Project Office (L&RPO), as recorded on the form, finalize the user's access journey, marking the provision or denial of permissions within the BLM's application security system.
