How to fill out Form H1020, box by box
Date, Advisor, Office Address and Client
Enter the date Form H1020 is completed, the advisor's name, office address and phone number in the upper right corner.
Enter the Name and Address of Client.[2]
Appointments
If the reason is an appointment: complete the section that tells the client they have an appointment, or complete the section that tells the client they missed an appointment and the final date they have to contact the office.
After "You have an appointment on", enter the Date and Time, and after "at", enter the Place.
The form tells the client to call if the client cannot keep this appointment.
After "You missed your appointment on", enter the Date and Time, and after "at", enter the Place.
After "Please call your local office by", enter the Date by which to schedule another appointment.[2]
Includes Federal Tax Information
If the form contains Federal Tax Information, check the box and follow procedures for safeguarding the information.[2]
Information Needed
After "Please return the following by", enter the Date; in the box under it, list the specific information the client needs to provide.
After "no later than", enter the Date by which the office must receive all information necessary to make a final decision on the client's eligibility. The form tells the client that if the office does not receive the information or hear from the client by the date after "Please return the following by", it may be necessary to deny the case.
Attach the page(s) of Form H1020-A, Sources of Proof, that correspond(s) to the verification requested.
The form tells the client: If you need help getting any of the items, call 2-1-1 or 877-541-7905.[2]
