The MCIR-150 form, a crucial document issued by the Maryland Department of Transportation Motor Carrier Division, serves as the backbone for motor carriers operating within the state. Essential for ensuring compliance with both federal and state regulations, this form plays a pivotal role in the transportation sector. It is designed for a variety of purposes including new applications for a Maryland DOT number, biennial updates, and re-application following a revocation of New Entrant status, among others. The form requires comprehensive information about the motor carrier or hazardous material shipper, such as company name, trade name, principal office address, and contact information. It also delves into the specifics of the operation, asking for details on the types of cargo transported, the operation classifications, and the types of hazardous materials carried or shipped. This detailed reporting ensures that carriers are properly tracked for safety and regulatory compliance. Motor carriers engaged in interstate commerce must possess a U.S. DOT number, while those operating intrastate must display a Maryland DOT number. The MCIR-150 form is not only a requirement for starting intrastate operations but must also be updated biennially to maintain accurate records with the Motor Carrier Division. This complex yet necessary form reflects the state's commitment to maintaining high safety standards within its transportation network.
