Form T 216 PDF Details

In the realm of vehicle ownership and administration, the journey towards ensuring all necessary legal documentation is properly accounted for can sometimes encounter unexpected obstacles, such as the loss of important documents in transit. This concern is effectively addressed by the T-216 form under the laws of Georgia, specifically designed to alleviate the distress caused when a Certificate of Title for a vehicle goes missing before it reaches the intended recipient. Codified under Section 40-3-31, paragraph (5), of the Official Code of Georgia Annotated (O.C.G.A.), the form allows vehicle owners to apply for a replacement title without being charged the usual $8.00 processing fee, provided their application is submitted within a 60-day window from the original title's issuance date. This application needs to be directed either to the County Tag Agent or the State Motor Vehicle Services, alongside a filled affidavit that attests to the loss of the title, backed by a solemn affirmation made under the risk of severe penalties for misinformation. This provision stands out as a thoughtful measure accommodating the mishaps that can occur in the mailing process, requiring the form to be completed with precision and honesty, ensuring that all specified fields are accurately filled to avoid invalidation. Moreover, the affidavit emphasizes the commitment to integrity, with the signatory(ies) vowing under penalty of perjury that their statements are true, a safeguard that underscores the gravity with which such applications are treated within the legal framework of Georgia's motor vehicle administration.
