Hsmv 61518 Form PDF Details

The Florida Highway Patrol's HSMV 61518 form serves as a crucial document for carriers operating within and across Florida's borders. It functions as an application for obtaining a Florida Department of Transportation (DOT) Number, a mandatory identifier for commercial vehicles enforcing compliance with safety regulations. The form collects comprehensive data on the carrier, including legal and trade names, contact details, operational metrics, and the specific nature of the cargo transported, which can include hazardous materials. Furthermore, it details vehicle information, driver statistics, and confirms adherence to safety and hazardous materials regulations, ensuring that carriers operate responsibly. By requiring carriers to disclose their operations' scope—interstate versus intrastate, cargo classification, and hazardous material transportation—the form acts as a regulatory tool for the Commercial Vehicle Enforcement division of the Florida Highway Patrol. It is designed to streamline the process of monitoring commercial vehicle operations, thus enhancing road safety and environmental protection. The necessity for carriers to provide detailed information about their operations, including the types of vehicles used and driver details, emphasizes the state's commitment to maintaining high transportation standards. Additionally, the inclusion of certification statements by authorized officials underscores the legal responsibility carriers bear in providing truthful and accurate information, aligning with federal and state safety regulations.
