INSTRUCTIONS ON WAIVER CERTIFICATE/NO OBLIGATION
TO RETURN TO INDIA CERTIFICATE
(For holders of
STAGE I
1.Make four photocopies of the enclosed form and affidavit.
2.Fill in original and have them notarized.
3.Provide complete details with clear and accurate answers. Do not use any vague expressions or dots and dashes. Do not leave any column blank.
4.Send four sets of these notarized forms with your passport and a fee of $60.00 in the form of Money Order or Certified checks for attestation to the Consular Section, Consulate General of India 3 East 64th Street, New York, NY – 10065.
STAGE II
On receipt of attested forms from the Consulate General of India, send one copy each to:
1.The Regional Passport Office (where your passport was issued initially)
2.The Home Department of State in India you belong to.
3.The Ministry of Human Resource Development, Department of Education,
4.The Ministry of Health and Family Welfare, Department of Health, Nirman Bhawan, New Delhi – 110011 (for Medical Professionals only)
STAGE III
On receipt of clearance from all three offices mentioned above, send a photocopy of attested set of form and affidavit with your passport, clearances received, and a fee of $20.00 along with mailing fee of US $ 10 for Priority Mail or US $ 20 for USPS Express mail. (in the form of Money Order/Certified check) to the Consular Section, Consulate General of India, New York, for further processing. Please also clearly indicate your CASE NUMBER given by US Immigration authorities. The Consulate General shall issue the waiver certificate and forward it to the Embassy of India in Washington DC for onward transmission to the US Immigration Authority who will notify the applicant directly regarding its acceptance.
ADDITIONAL CHECK LIST FOR JI WAIVER
BIODATA AND AFFIDAVIT FORM :
I. The document should be in the prescribed format.
II.These documents should be sent to all the prescribed addressees in original. (Please check Instructions for Stage II for the addressees).
III. These should bear the signature of the applicant, Notary Public, Consulate General of India, in Blue Ink only.
IV. The applicant should give specific replies to all the paras of the
EDUCATIONAL OUALIFICATIONS:
I. In case the applicant has mentioned the Ph.D degree only, he should attach copies of graduation, post graduation, and Ph.D degrees.
II.In case the applicant has taken loan from any agency/institution under the National Loan Scholarship scheme or any other scheme during the year, he/she should attach N.O.C. from the concerned agency/institution.
