In the realm of building safety and maintenance within Kentucky, the EV-7 form stands as a crucial document, guiding the process of elevator license renewal under the vigilant oversight of the Commonwealth's Public Protection Cabinet, Department of Housing, Buildings, and Construction, specifically within the Division of Building Codes Enforcement. This comprehensive form mandates detailed information from applicants such as personal details, employer information, and, notably, the license type — whether for contractors, elevator mechanics, or accessibility and residential elevator mechanics. With the requirement of a non-refundable application fee made payable to the Kentucky State Treasurer, the form underscores the state's commitment to maintaining stringent safety standards. Additionally, it delineates both the renewal fees and additional charges that may incur due to late renewals, alongside the enforced stipulations for license restoration or reinstatement post-expiration. Furthermore, the EV-7 form elaborates on prerequisites for renewal, like the necessity of continuing education, liability and Worker’s Compensation insurance for contractors, and even a background check for certain applicants. This document not only serves as a gateway for professionals to continue their practice but also as a testament to Kentucky's rigorous approach towards upholding safety and professional integrity in the elevator industry.
