The LIC311A form is a comprehensive document used by the State of California Health and Human Services Agency, specifically within the California Department of Social Services Community Care Licensing Division. This form is vital for child care centers, infant centers, school-age centers, and child care centers for mildly ill children, as it outlines the records that must be maintained at the facility. These records are essential for regulatory compliance under sections of Title 22, California Code of Regulations, and/or Statute, ensuring that all mandatory information is kept complete, current, and available for review. The form covers a wide array of topics, from child and personnel records to administrative records and documents that must be visibly posted at the facility. Child records include identification, emergency information, health history, and consent for medical treatment, among others. Personnel records focus on health screening, qualifications, criminal background checks, and rights acknowledgment. Administrative records emphasize operational protocols like emergency disaster plans and licensing reports. Additionally, the form dictates specific documents that must be posted within the facility for public view, ensuring transparency and accountability in child care services. Thus, the LIC311A form plays a crucial role in maintaining the standards of care and safety in child care facilities throughout California.
