Public Guardian registers
This form is to ask the Ofice of the Public Guardian whether:
•an Enduring Power of Attorney (EPA) has been registered;
•a Lasting Power of Attorney (LPA) has been registered; and/or
•a court appointed Deputyship order has been made in relation to a particular person.
