Instructions: 1) A conservator of the person may use this form to report to the Probate Court at least annually on the condition of the person under conservatorship. If there is no conservator of the person appointed, the conservator of the estate may use this form to file a report with the court one year after a conservator was appointed and every three years after the initial appointment as part of the statutory review of the conservatorship.

2)Upon termination of a temporary conservatorship, the temporary conservator may also use this form to file a report with the court. For more information see C.G.S. sections 45a-654 (f), 45a-656 (c) and 45a-660 (c).

3)The conservator should respond to every question. If a question does not apply, the conservator may indicate "Not Applicable" or "N/A."
