To make it more convenient for Individually Paying Members (IPMs) and employers, both from private and government sectors to remit their premium contributions through accredited collecting agents (ACAs), the PhilHealth Premium Payment Slip (PPPS) may now be downloaded from http://www.philhealth.gov.ph/forms/others/ppps.pdf.

The PPPS is a non-accountable form and may be reproduced freely. It serves as the ACAs’ basis when filling out the PhilHealth Agents Receipt (PAR) which is validated by the ACA representative once payment is made.

Except for Unionbank, United Coconut Planters Bank and Allied Bank, which will continue using the contribution payment return forms (CPRs) until March 31, 2011, all other ACAs are now using the PPPS.
