What it is: This form is the Secretary of State Vehicle License Plates Revocation Request.
Where and when to file: Secretary of State Vehicle Services Department Special Plates Division, 501 S. Second St., Springfield, IL 62756, Platerevocations@ilsos.gov.
To request a revocation, you must provide the correct owner(s) and vehicle information, reason for revocation and signature(s) of all registered/titled owner(s). If the vehicle is leased, the lessor must also sign. Revocation of the license plate does not remove your name from the vehicle record.

How to fill out Form VSD 851, box by box
License Plate Number and Vehicle Identification Number
Enter the License Plate Number, Vehicle Identification Number, Vehicle Make and Vehicle Year.
Last Name/1st Owner and Last Name/2nd Owner
Enter the Last Name, First Name and Middle Initial of the 1st Owner, and, if there is a 2nd Owner, of the 2nd Owner.
Current Address, phone and driver's license
Enter the Current Address, Apt.#, City, State and ZIP Code.
Enter the Phone Number(s) and Driver's License Number(s).
Reason for License Plate Revocation (check one)
Under Reason for License Plate Revocation (check one): Vehicle sold or traded in with license plates attached; License plates lost, missing or destroyed; Registered owner deceased (copy of death certificate required); Divorce (single owner only; copy of divorce decree required); License plates stolen (copy of police report required).
Also: Registered owner moved out of state; Vehicle towed or junked with license plates; Vehicle donated to charity with license plates; Court order (Attach copy); Vehicle repossessed with license plates.
Replacement at no fee for a stolen license plate or sticker
Indicate if a license plate and/or sticker has been stolen and a replacement is needed at no fee according to 625 ILCS 5/3-820: Theft of license plates and sticker, or Theft of sticker only.

Under penalties as provided according to Section 1-109 of the Code of Civil Procedure, by signing this document, you confirm that the information is accurate, except for any parts you are unsure about; for those you believe them to be true. (735 ILCS 5/1-109)
