The UIA 1583, Record of Work Search, is a required Michigan Unemployment Insurance Agency document. It tracks the job search efforts of individuals claiming unemployment benefits under the oversight of the Department of Labor and Economic Opportunity. Claimants must submit accurate records for every week they certify for benefits.
Each week's record must include at least two employer contacts. For each contact, claimants must document the date, the employer's name and address, the contact method (in person, by phone, by mail, by fax, or through an online application), the type of work sought, and the result. Missing or incomplete entries can delay or stop benefit payments.
Completed records can be submitted through the MiWAM online account or retained in an alternative format such as a spreadsheet or handwritten notes. All records are subject to audit by the UIA. Providing inaccurate work search information can result in repayment of all benefits received, additional financial penalties, and potential criminal prosecution under Michigan law.
