Domestic relations history: Disclose any domestic violence restraining orders, protective orders, or misdemeanor domestic violence convictions, regardless of whether the sentence has been served or the conviction set aside.
Character references: List three or more individuals who can attest to your rehabilitation and responsible character. References must not be family members and should have known you for an extended period.
Certification and signature: Read the certification statement carefully before signing. Your signature certifies that all information in the application document is true, complete, and correct. False statements on this federal form constitute a criminal offense.
