The AWS Visual Acuity Form is required for all SCWI, CWI, CAWI, CRI, and CWEng certification applicants from the American Welding Society. Without this completed form on file, the AWS will not release exam results, recertification results, or certificate renewals.

Visual Acuity and Color Perception Standards
