The AWS Visual Acuity Form is required for all SCWI, CWI, CAWI, CRI, and CWEng certification applicants from the American Welding Society. Without this completed form on file, the AWS will not release exam results, recertification results, or certificate renewals.

The form documents the results of a near vision acuity test using the Jaeger J2 chart. The test is performed at a minimum distance of 12 inches. Applicants may use corrective lenses to meet this standard. A color perception test is also required as part of the full examination process.
A licensed medical professional must conduct the eye examination and sign the completed form. Accepted examiners include Ophthalmologists, Optometrists, Medical Doctors, Registered Nurses, and Certified Physician Assistants. The examiner certifies that the applicant meets the AWS vision standards.
Submit the completed form before your certification exam when possible. For CAWI, CWI, and SCWI candidates, the deadline is within 60 days after the certification exam. All other programs that require a Visual Acuity Form must submit within 30 days. Missing these deadlines can result in voided test scores and forfeiture of application fees.
