Group Release
The Beneficiaries represent and declare to BCLC by initialing in the Declaration Box below that each of the Beneficiaries to the best of his or her knowledge is not a member of the following listed categories:
a.A Lottery Retailer. Lottery Retailer is defined as a person who operates a retail location contracted by BCLC to sell lottery tickets and provide lottery services or, a person who is an employee or volunteer of a retail location contracted by BCLC and is authorized to operate a lottery terminal or is involved in the handling, selling and/or validation of lottery products.
b.A family member of a Lottery Retailer. A family member of a Lottery Retailer is defined as a person who is a grandparent, grandchild, aunt, uncle, nephew, niece, cousin, parent, child, sibling of a Lottery Retailer (whether related biologically or through marriage), a spouse (including common law), or anyone living at the same residence of a Lottery Retailer.
c.A BCLC employee.
d.A family member of a BCLC employee. A family member of a BCLC Employee is defined as a person who is a spouse (including common law) or sibling (whether related biologically or through marriage) living at the same residence of a BCLC Employee.

Group Release
The Beneficiaries further represent to BCLC that if he or she belongs to one of the above listed categories, BCLC will follow a different prize payout procedure with respect to his or her claim for a share(s) of the prize.
The Beneficiaries further represent to BCLC that the Group Trustee is the holder of the ticket as trustee for the Beneficiaries, and instruct and authorize BCLC to deliver any and all prizes won on the Ticket (“Prize”) to the Group Trustee on behalf of the Beneficiaries.
In consideration of and to induce BCLC to deliver the Prize to the Group Trustee, the Group Trustee and the Beneficiaries represent, warrant and agree with BCLC that:
1.The Beneficiaries have read, are familiar with and agree to be bound by the “Rules and Regulations Respecting Lotteries and Gaming” of BCLC and the definitions therein shall be applicable to this Agreement.
2.Payment or delivery of the Prize to the Group Trustee will release BCLC from any further claims or demands by the Group Trustee and the Beneficiaries in respect of the Ticket and the Prize.
3.All parties entitled to the Prize have been identified as a Beneficiary in this agreement. All Beneficiaries acknowledge and confirm that BCLC has no responsibility to ensure receipt of any Prize or portion thereof by any Beneficiary.
4.The Beneficiaries are each the full age of nineteen (19) years.
5.The Group Trustee and the Beneficiaries will indemnify and save BCLC harmless from and against any and all actions, claims or demands (including costs incurred by BCLC in defending such actions, claims or demands) brought against BCLC with respect to the Ticket or payment or the delivery of the Prize or any portion thereof.
6.The Group Trustee and the Beneficiaries grant BCLC the irrevocable right to publish and broadcast the Group Trustee’s and Beneficiaries’ names, addresses and photographs in any and all manner and medium as BCLC may determine without any liability to the Group Trustee or Beneficiaries for doing so.
