Tips for Accurate Completion
Double-check all names, numbers, and addresses before submitting. Use the exact legal name registered with the Oregon Secretary of State. Even small errors can cause processing delays.
If you are reporting an ownership change, list all new and departing owners with their contact details. For business closures, include the final date you paid wages in Oregon. Allow 2 to 4 weeks for the Oregon Department of Revenue to process your submission. You will receive written confirmation once the changes take effect.
If your business closes permanently, also contact the Oregon Employment Department separately. Closing your payroll tax account does not automatically close your unemployment insurance account. You must notify each agency directly.
