The Connecticut SBA-2 form is the official CPA Certificate Registration Application for the Connecticut State Board of Accountancy. CPAs use this form to register for limited, legal use of the "Certified Public Accountant" title and the CPA initials in Connecticut, without the authority to practice public accountancy.
Last revised in February 2012, the form is issued by the Office of the Secretary of the State. Completing it requires a non-refundable fee of $40.00 paid by check or money order. Applicants must enter their full name and mailing address as they wish them to appear on official state records, along with their CPA Certificate Number and the issuing jurisdiction.
After registration, the CPA title may be used for personal purposes, by employees of CPA or PA firms (but not proprietors, partners, or shareholders), by officers or employees of non-public-accountancy businesses, and by academic faculty or administrators. Registration does not authorize the holder to practice public accountancy.
