The CS Form 212 Revised is the official Personal Data Sheet (PDS) for Philippine government employees and applicants. It is one of the most commonly required documents for civil service employment in the country.
Government agencies require a completed CS Form 212 Revised for hiring, promotion, transfer, and other HR transactions. The form must be updated and resubmitted whenever an employee assumes a new position. It must be signed under oath before a notarizing officer to certify that all entries are accurate and complete.
The form is divided into several parts: personal information, family background, educational background, civil service eligibility, work experience, voluntary work, and learning and development (trainings attended). Each part requires detailed and accurate information. False entries may result in disqualification or dismissal from government service.
When submitting a printed copy, attach two passport-size photos with your signature across the front. Include a photocopy of a valid government-issued ID.
