Part II rates the service member across a detailed set of performance factors using a structured scoring scale. Factors evaluated include:

• Oral expression and writing ability
• Confidence in own knowledge
• Accepting responsibility and motivating others
• Working relationships and adaptability

The RATER COMMENTS section allows the evaluating supervisor to add written feedback specific to the individual. A final calculation produces an overall evaluation score from the Part II ratings. Both the rater and a reviewer must print their names, note their titles, enter the date, and sign the form. This dual sign-off ensures accountability and formal acknowledgment of the results.
