The CSRS Standard Form 2808, also called the Designation of Beneficiary form, is used by federal employees and retirees covered under the Civil Service Retirement System. It lets you name one or more beneficiaries to receive any lump-sum benefits payable after your death. Filing this form is the only way to override OPM's default payment order.

Key facts about SF 2808:

• It replaces all prior CSRS beneficiary designations. The most recent valid form on file is the only one OPM will honor.
• Each beneficiary must be identified by full legal name, current mailing address, Social Security number, date of birth, and relationship to you.
• You must assign each beneficiary a percentage share of the total benefit. All shares must add up to exactly 100%.
• This form applies only to lump-sum death benefits. Survivor annuity benefits are governed by separate OPM rules.
• A beneficiary named on the form cannot act as a witness. You need two independent adult witnesses.

If you do not file SF 2808, OPM distributes lump-sum benefits in this default order: your widow or widower, children (including legally adopted children), parents, the executor of your estate, next of kin under state law, and unnamed heirs. Filing the form lets you override this sequence entirely.

Update your SF 2808 after any major life change. Marriage, divorce, the birth or adoption of a child, the death of a named beneficiary, or a change in a beneficiary's address are all reasons to file a new form. Submit it to your human resources office or directly to OPM. A new form takes effect as soon as OPM receives it.
