The CY 47 Form is Pennsylvania's mandatory written child abuse report for all mandated reporters. After calling ChildLine (1-800-932-0313) to report suspected child abuse or neglect, you must submit this form to the county Children and Youth agency within 48 hours of the oral report.
EMS providers, emergency room nurses, school counselors, and other professionals are all mandated reporters in Pennsylvania. Each person who makes a separate ChildLine call about the same family must file their own CY 47. Fill out every section you can, even when details about the family are limited. The reporter's contact information stays confidential and is only disclosed to law enforcement when the investigation requires it.
After submission, the county agency reviews the report and opens a child abuse investigation. If you have not heard back within 30 days, you may contact the agency to ask about the case outcome and any services provided to the family.
