Step 1: Enter Team Identification Information
Begin at the top of the form with the team name and team number. These fields identify which unit is competing in the match. Write the full official team name as it appears in your unit records. Enter the correct team number assigned for the event. This information is used to organize competitors within a large match with multiple teams.
Step 2: Record Team Captain and Coach Details
Fill in the printed name of the team captain and the team coach in the appropriate boxes. Both individuals must also provide their signatures on the form. Contact information, including a phone number or email, may be required for follow-up by range officials. Verify that the team captain and coach listed are the individuals who were present during the match.
Step 3: Complete the Eligibility Statements
The DA Form 1343 includes eligibility checkboxes for each category of match. These categories may include Regular Army, Reserve Component, National Guard, and others. Check the box that applies to your team's category. Incorrect eligibility classification can result in disqualification from official results. Review Army Regulation 920-30 if you are unsure which category applies to your team.
Step 4: Enter Match Details
Write the type of match in the designated field near the top of the form. Record the exact location of the event, including the range name if required. Enter the date of the competition in the correct format. Fill in the squadding relay number that applies to your team. Also record the target type used during the match, as different match categories use different target specifications.
Step 5: Record Individual Scores
List each team member's full name and military grade in the roster section. Record their scores for each of the three firing stages: slow fire, timed fire, and rapid fire. Write the score for each stage in the matching column. Add up the stage scores to get each member's total score. Enter the total in the column for overall individual score. Check each entry against the range officer's score sheet before recording it on the form.
Step 6: Calculate and Record the Team Total
Add up all individual totals to determine the team's combined score. Write this figure in the team total field at the bottom of the form. If a team member did not fire a particular stage, leave that stage score blank. Only enter a zero if the competitor was present and fired a zero. Entering a zero for an absent member changes the team total and may affect official placement.
Step 7: Obtain Required Signatures
The team captain must sign and print their name in the designated signature block. The team coach must do the same in the coach's signature block. Both signatures are required for the form to be considered official. A form without the required signatures will not be accepted by range officials. Collect all signatures at the completion of the match before leaving the range.
