The Transmittal Record is a document management tool used throughout the Army chain of command to log the movement of sensitive and standard materials. Soldiers, administrative personnel, and unit clerks complete this form when transferring documents, media, or equipment between offices, units, or installations.
The form captures the security classification of all materials being transmitted, the shipment number assigned to the transfer, and the full identity of both the sending and receiving parties. Personnel must specify the type of media: hard copy documents, punched cards, cassettes, photo microfilm, or fiche. The method of shipment (courier, express mail, or first class) is recorded along with the number of boxes and individual items.
The sending officer provides a signature and cites the authority under which the shipment is authorized. The receiving party countersigns upon receipt, creating a two-party accountability record. This dual-signature structure ensures that sensitive materials cannot be transferred without acknowledgment on both ends, supporting Army security and records management policy.
