Dmv Form Ti 008 PDF Details

In the realm of vehicle ownership and maintenance, accurate records are essential, particularly when it pertains to the odometer reading which reflects the mileage of a vehicle. The South Carolina Department of Motor Vehicles offers a solution for correcting inaccuracies in this vital area through the Odometer Correction Request, Form TI-008. This form serves a crucial purpose for vehicle owners who discover discrepancies between their vehicle's actual mileage and what is recorded on their title. It outlines a process for requesting an official correction, requiring detailed information about the vehicle, including the Vehicle Identification Number (VIN), make, model, and year, alongside personal details of the requester. Importantly, it emphasizes the necessity of providing original documents to substantiate the claim that the odometer reading on the title is in fact incorrect. Suggested documents include repair records, oil change records, and statements from previous owners, though it is noted that the absence of such documentation could impede the correction process. The form also outlines different scenarios under which a correction request can be made, whether due to errors by the department or the vehicle owner, and specifies the steps to follow, including a fee for the title change. Furthermore, it warns that even with submitted proof, not all requests will result in an “Actual Mileage” designation on the corrected title, introducing complexities in cases where conclusive evidence cannot be presented. Finally, the form closes with a section for official use, including declarations under penalty of perjury by the requester and acceptance by a DMV employee, highlighting the seriousness and legal implications of submitting such a request.

South Carolina Title No. ___________________________ was issued on _______________, showing an odometer reading of

___________________________. The application for that title (SCDMV Form 400) showed that the actual mileage on the

The correct odometer reading on the date of the application was _______________________.

The incorrect reading was put on the title because:

_______________________________________________________________________________________

_______________________________________________________________________________________

(USE EXTRA SHEETS AS NEEDED)

IMPORTANT NOTE: Original documents must be provided to prove to the Department’s satisfaction that the odometer reading printed on the title was incorrect and that the new mileage accurately records the odometer reading on the original date of application. The following documents should be submitted, if available. (The lack of documentation will likely result in the Department’s inability to change the title.)

A series of repair records that show the vehicle mileage.

A series of oil change records that show the vehicle mileage history.

Statements from previous owners certifying mileage of the vehicle. These statement cannot be used to certify the actual mileage of the vehicle and will require an indication of “Warning – not actual mileage” be added to the title.

Please note that all odometer corrections will not indicate an odometer legend of “ACTUAL MILEAGE”. If the request for correction does not prove to the department’s satisfaction that an error was made, the title will be marked “WARNING – NOT ACTUAL MILEAGE”.

Under penalties of perjury, I declare that I am the owner or acting on behalf of the owner and request that a corrected South Carolina Certificate of Title be issued. I further certify that the information on this request is correct to the best of my knowledge.

Requests for odometer corrections made because of Department error will be corrected and may be requested at any DMV Full Service Branch office.
