How to fill out Form TI-021A, box by box
Vehicle identification number, make, year, body & style, model, empty weight and gross vehicle weight (GVW)
Enter the VEHICLE IDENTIFICATION NUMBER, MAKE and YEAR.
Enter the BODY & STYLE, MODEL, EMPTY WEIGHT and GROSS VEHICLE WEIGHT (GVW).
Section 2 - New primary owner
In Section 2, your complete name must be used on all title and registration documents.
Enter the NEW PRIMARY OWNER COMPLETE NAME (LAST, FIRST, MIDDLE), SC CUSTOMER NO.; DL NO.; SSN, OR FEIN and DATE OF BIRTH.
If you do not provide your SC customer number, SC driver's license number, or FEIN, then a social security number is required on this form to ensure the accuracy of DMV's records regarding this transaction.
New co-owner and shared ownership
If there is a new co-owner, enter the NEW CO-OWNER COMPLETE NAME (LAST, FIRST, MIDDLE), SC CUSTOMER NO.; DL NO.; SSN, OR FEIN and DATE OF BIRTH. SHARED OWNERSHIP (Required, if applicable): AND or OR.
Residence address
Enter the PRIMARY OWNER'S RESIDENCE STREET ADDRESS (APT. NO. IF APPLICABLE), CITY, STATE, ZIP CODE and COUNTY.
Mailing address (if different from above)
I understand the department will send mail to the residence address in the Residence address box above unless I have specified a mailing address in the Mailing address box below in Section 2. Enter the MAILING ADDRESS (IF DIFFERENT FROM THE RESIDENCE ADDRESS BOX), CITY, STATE, ZIP CODE and COUNTY.
Telephone number and email address
Enter the DAYTIME TELEPHONE NUMBER and EMAIL ADDRESS.
Update voter registration
Unless you indicate otherwise, the addresses in the Residence address and Mailing address boxes will be used by the State Election Commission to update your voter registration: Do not update my residence address. Do not update my mailing address.
Section 3 - Odometer mileage
Federal and state law requires that you state the mileage in connection with the transfer of ownership. Failure to complete or providing a false statement may result in fines and/or imprisonment.
In Section 3, miles not kilometers: I state that the odometer now reads (no tenths) miles and to the best of my knowledge that it reflects the ACTUAL MILEAGE of the vehicle described herein, UNLESS one of the following statements is checked.
DO NOT check one of the following unless it applies: The mileage stated is in-excess of its mechanical limits. (The odometer started at zero again.) The odometer reading is not the ACTUAL mileage. WARNING: ODOMETER DISCREPANCY.
Lien information (if applicable)
For LIEN INFORMATION (If applicable), enter the CUSTOMER NO. OR FEIN, LIENHOLDER NAME, DATE OF LIEN, CONTACT PERSON and PHONE NUMBER; an ELT provider must include the ELT customer Number.
For LIEN INFORMATION (If applicable), enter the MAILING ADDRESS, CITY, STATE and ZIP CODE.
Purchase price, date of purchase and why previous certificate of title was not obtained
Enter the PURCHASE PRICE and DATE OF PURCHASE.
Explain why previous certificate of title was not obtained. Explain in detail how, where, and when you acquired the vehicle, who was involved, the condition of the vehicle, and any other information regarding the vehicle.
Sign with all listed owners
Warning: Fraudulent application and falsification of documents is a felony under South Carolina law. Under penalties of perjury, I declare that to the best of my knowledge and belief, this statement is true, correct, and complete. I further state that to the best of my knowledge, the vehicle is free and clear of any liens, encumbrances, lawful claims, demands of any person, and is not involved in any existing or pending litigation. I agree to indemnify the South Carolina Department of Motor Vehicles and all persons acting under direction of the Department, from any and all liability and shall defend all litigation that may arise as a result of the issuance of a South Carolina certificate of title in my name.

Additional Requirements
No unique identifying number on the vehicle
If a unique identifying number is not affixed to the vehicle, before proceeding with this request, the customer will be required to apply for an assigned serial number from the Department by completing Application for an Assigned Serial Number (Form 401-A).
Additional Requirements for Mobile Homes
For a mobile home: A letter or receipt from the county treasurer's office indicating property taxes are current for the mobile home. Seven (7) years of property tax statements in the current owner's name; or, insurance company records indicating the home has been insured in the owner's name for the previous seven (7) years or more. The insurance documents or property tax statements may also be used to indicate ownership rights of prior owners or the person(s) selling the vehicle, as well as former deeds or installment sales contracts or bills of sale identifying the mobile home by make, model year, and VIN.
Before you go
To receive a title for a vehicle, trailer, or mobile home where no previous title is available, the current owner of the vehicle, trailer, or mobile home must submit the following.
Affidavit of Ownership for a Motor Vehicle, Trailer, or Mobile Home (Form TI-021A) to include the vehicle, current owner information, and a statement detailing how, where, and when the owner acquired the vehicle, who were the seller(s) and/or lien holder(s), the condition of the vehicle and any other information regarding the vehicle.
Photographs of the vehicle from all four sides (photos will not be returned).
Copy of the owner's out of state credential, passport, employment authorization document or permanent residency card along with a completed Statement of Vehicle Operation in South Carolina (Form TI-006).
Vehicle Identification Verification for Title without Previous Title (Form TI-021B), from a SCDMV official. The vehicle must be inspected by a Dealer License and Audit Agent or a member of Law Enforcement.
