Instructions for Filing Monthly Employment Utilization Report

(CC-257)

The Monthly Employment Utilization Report (MEUR) is to be completed by each subject contractor (both Prime and Sub) and signed by a responsible official of the company. The reports are to be filed monthly with a Weekly Payroll Report during the term of the contract, and they shall include the total work hours for each employee classification in each trade in the covered area (City of Madison) for the period covered. All reports shall be submitted to the City of Madison Affirmative Action Division. Additional copies of this form may be reproduced or obtained from the Affirmative Action Division at (608) 266-4910.

Note: The PRIME CONTRACTOR submits a report for its total construction workforce to the Affirmative Action Department. Each SUBCONTRACTOR separately submits a report for its total construction workforce to the Affirmative Action Division.
