This self-certification affidavit applies solely to two-wheeled motorbikes that have been converted by the titled owner for operation on all public roadways - see Idaho Code 49-114 (10). The conversion must meet the current Federal Motor Vehicle Safety Standards (FMVSS) in effect at the time of application, and also meet current Idaho Title 49, Chapter 9 requirements for operation on all public roadways.

I certify the above motorbike has been converted to meet the following Federal Motor Vehicle Safety Standards (FMVSS) required for motorcycles operated on US public roadways. I accept all legal responsibilities for ensuring this vehicle meets or exceeds all US DOT standards listed under the Title 49 CFR, Part 571, FMVSS requirements for motorcycles operated on public roadways.

The pertinent FMVSS standards are: 106-Brake Hoses; 108-Lamps; Reflective Devices and Associated Equipment; 111- Rearview Mirrors; 116-Brake Fluids; 119-Tires; 120-Tires and Wheels; 122-Brake Systems; 123-Controls and Displays.
