Mdj Form 270 PDF Details

Traveling internationally with pets involves a myriad of rules and regulations, especially for those moving to Japan under the auspices of the U.S. Forces. The MDJ 270 form, also known as the Pet Quarantine and Examination Certificate, is an essential document for military personnel seeking to bring their pets into Japan, addressing these very requirements. Japanese law mandates up to a 180-day quarantine for pets entering the country, but for those belonging to U.S. Forces personnel, there’s an option of a "Working Quarantine" that allows pets to stay with their owners under certain conditions. This arrangement necessitates a disciplined adherence to identification, vaccination, and examination prerequisites, ensuring pets are kept within prescribed environments and monitored for health issues. Additionally, the form underscores the legal obligations owners undertake to comply with these standards, including the potential financial repercussions of non-compliance. By laying out procedures from arrival to final examination, the MDJ 270 form encapsulates a critical step in facilitating a smooth transition for military families and their pets, emphasizing the importance of preparedness and responsibility in international pet relocation. Its implementation not only safeguards pet and public health but also integrates veterinary oversight into the relocation process, ensuring that pets enter Japan with a clean bill of health.

PART I – CERTIFICATE OF COMPLIANCE

Japanese law requires pets entering Japan to be quarantined up to 180 days. Pets belonging to U.S. Forces personnel may be placed in a “Working Quarantine” in the custody of the owner under the supervision of the U. S. Forces veterinary service for onpost/base housing, providing all paper work has been properly completed. Quarantine length depends on proper identification, rabies vaccine history, Fluorescent Antibody Viral Neutralization Test (FAVN) results, date of FAVN testing, and appropriately prepared and verified documents. You must: 1) keep your pet in travel carrier until arrival at military installation; 2) report to the U. S. Forces veterinary clinic within 72 hours, or the first clinic day following arrival of the animal to Japan; 3) present your animal to the U. S. Forces veterinary clinic within 72 hours of completion of the quarantine requirement.

In accordance with the above, I request the pet(s) described below be placed in quarantine in my custody under the supervision of the U. S. Forces veterinary service. I will present my pet(s) to the U.S. Forces veterinary clinic as prescribed and immediately notify vet clinic if my pet exhibits unusual behavior, becomes ill or dies. I will keep my pet(s) within my household on U. S. Installation at all times during the quarantine period. I will not allow my pet(s) to come in contact with other animals or people that are not members of my immediate family.

I fully understand and will comply with above regulations, including all quarantine requirements and understand that any failure to do so will result in forwarding a copy of this report through command channels and a fine of up to 300,000 yen per animal (USFJI 90‐205).
