Write the Defendant's Birthplace (State or Foreign Country) and the Number of this Marriage (First, Second, etc. - Specify).
The marriage and the date the couple last resided in same household
Write the Place of this Marriage in the City, Village, or Township, the County and the State or Foreign Country.
Write the Date of this Marriage (Month, Day, Year).
Write the Date Couple Last Resided in Same Household, or check Check if Not Separated.
Minor children
Write the Number of Minor Children in Household at Separation Date (Filing Date if Not Separated) (Number), or check Check If None.
Plaintiff's Attorney
The Plaintiff's Attorney writes the Name - Type or Print and the Bar Number.
Write the Attorney's Address in the Number and Street, the City, the State and the Zip Code.
Judgment of Divorce or Annulment and physical custody
Write the Judgment of Divorce or Annulment (Specify if Divorce or Annulment) and the Number of Minor Children Whose Physical Custody was Awarded to the Plaintiff, the Defendant, Joint or Other, writing each Number; check No Children or Unknown.
Certifying Official
The Certifying Official writes the Judgment Recorded on (Month, Day, Year), certifies that this Divorce was granted on (Month, Day, Year), signs on the Certifying Official line with the Signature and writes the Title and the Date Signed.
