Follow the steps below to complete your odometer disclosure statement properly.
1. Notary Information
Leave the first line blank. The notary public will fill in their name, state, and county where the notarization takes place.
2. Mileage Entry
Write the exact mileage shown on the vehicle’s odometer at the time of signing. Don't round the number or estimate.
3. Car Details
Fill in the car’s year, make, model, and full VIN exactly as they appear on the title or registration.
4. Owner Name
Write the full legal name of the registered owner in the space provided. This name must match the name on the title.
5. Date of Notarization
The notary will complete the date when the owner appears and signs the form.
6. Owner Signature
The registered owner must sign the form in front of the notary. Don't sign it in advance!
7. Notary Signature and Commission
The notary signs, applies their seal, and fills in their commission expiration date.
