False statement: failure to complete or providing a false statement may result in fines and/or imprisonment.
Buyer: the section To be completed by the buyer holds the buyer's signature, printed name, address and acknowledgment date.
Vehicle: Year, Make, Body Type, Model and Vehicle Identification Number.
