Request For Osap Access Number Form PDF Details

The journey to further education can be filled with excitement and, at times, complex administrative processes. One such step for students in Ontario seeking financial assistance through the Ontario Student Assistance Program (OSAP) involves the Request for OSAP Access Number (OAN) form. This crucial document is the gateway for students who are either unable to access the secure side of the OSAP website due to a forgotten or unknown OAN, or are first-time applicants requiring an OAN to proceed with their financial aid applications. The form itself comprises three sections: the first part captures the student's personal information, including their Social Insurance Number and contact details, which must be accurately filled to avoid processing delays. The second section requires verification from an authorized third party — designated professionals such as senior postsecondary school officials, notary publics, or senior officials at Canadian embassies, among others — to confirm the student's identity by reviewing valid proof of Social Insurance Number and government-issued photo ID. The final section is completed by the authorized third party, ensuring that the information provided aligns with the proof of identity shown to them. It's important that students include photocopies of the relevant documentation when they submit their forms to the Ministry of Training, Colleges and Universities for processing. With precise instructions for each step, the form lays the foundation for accessing various OSAP funding instruments, emphasizing the necessity of accurate and complete submissions for a seamless transition into student financial assistance.

Purpose of this Form

The OSAP Access Number (OAN) is a nine-digit number that is assigned to each person who has:

registered on the secure side of the OSAP website, and/or

submitted a paper application for various OSAP funding instruments (e.g., OSAP for Full-Time Students, Ontario Graduate Scholarship, Explore).

Once you are assigned an OAN, it becomes your unique identifying number that will not change over time.

The Request for OSAP Access Number (OAN) form is for students who are unable to access the secure side of the OSAP website because they have forgotten or don’t know their OSAP Access Number (OAN).

There are quicker ways to get access!

We strongly recommend that you try to log into the “Forgot Your OAN?” feature on the OSAP website. This feature is fast and easy and will provide you with your OAN after you answer a few questions (see details on log in page).

Did you know that you can also retrieve your OAN in person? Simply go to the nearest fi nancial aid offi ce at any provincially- funded university or college of applied arts and technology in Ontario (which doesn’t have to be for the school that you are or will be attending) to obtain your OAN. You must provide proof of your Social Insurance Number (SIN) plus one piece of government- issued photo identification.
